Plan Type

Please select how the participant’s NDIS funding is managed. This determines how services are booked and invoices are processed.

  • NDIA-Managed – The National Disability Insurance Agency pays providers directly.

  • Plan-Managed – A registered plan manager handles invoices and payments.

  • Self-Managed – The participant (or nominee) manages the funds and pays invoices directly

  • A copy of the participant’s NDIS Plan

  • Risk assessments or medical reports

  • Behaviour Support Plans

  • Support Coordination notes

  • Other relevant documentation
